Privacy Policy for the SAFEHealth App

Introduction
We recognize that the privacy of your personal information is important. The purpose of this policy is to let you know how we handle the information collected through the use of SAFEHealth App. This policy does not apply to information collected through other means such as by telephone or in person, although that information is protected by other Reliant Medical Group privacy policies. This SAFEHealth App gives you the opportunity to provide us with personal information about yourself. You do not have to provide us with personal information if you do not want to simply by not using the SAFEHealth App; you will continue to receive high quality care at Reliant medical Group.

• To monitor your weight so that we can identify significant gains or losses that may impact your health or be a sign of impending health problems.
• To monitor your blood pressure so that we can identify when it is significantly elevated or low such that it may impact your health or be a sign of impending health problems. • To monitor your heart rate so that we can identify when it is significantly elevated or low such that it may impact your health or be a sign of impending health problems.
• To monitor your body temperature so that we can identify when it is significantly elevated or low such that it may impact your health or be a sign of impending health problems.
• To monitor your oxygen level so that we can identify when it is significantly low such that it may impact your health or be a sign of impending health problems.
• To monitor your activity (steps, speed, location) so that we can identify significant changes that it may impact your health or be a sign of impending health problems.
• To monitor your diet so that we can identify significant changes that it may impact your health or be a sign of impending health problems.
• To monitor your sleep patterns so that we can identify significant changes that it may impact your health or be a sign of impending health problems.
• To perform analytics to determine the effectiveness of the SAFEHealth App and to improve our services.
• To comply with applicable laws, regulations, and legal process.
• To protect someone's health, safety, or welfare.
• To protect our rights, the rights of affiliates or related third parties, or take appropriate legal action, such as to enforce our Terms of Use.
• To keep a record of our transactions and communications.
• As otherwise necessary or useful for us to conduct our business, so long as such use is permitted by law.
We may use personal information obtained using the SAFEHealth App to contact you about your health through any contact information you provided to your Reliant healthcare team, including any mailing address, email address, telephone number, cell phone number, text message number, or fax number.
Sharing Personal Information
We will share your personal information with your healthcare team at Reliant Medical Group in order to provide you with higher quality, safer, more convenient healthcare. We will only share your personal information with third parties as outlined in this policy and as otherwise permitted by law.
We may share personal information if all or part of the Company is sold, merged, dissolved, acquired, or in a similar transaction.
We may share personal information in response to a court order, subpoena, search warrant, law or regulation. We may cooperate with law enforcement authorities in investigating and prosecuting activities that are illegal, violate our rules, or may be harmful to other visitors.
We may also share personal information with other third party companies that we collaborate with or hire to perform services on our behalf. For example, we may hire a company to help us analyze your information to help us identify significant abnormalities or trends so that we can provider better care to you.
